Medical Form Process

A Camp IAWAH Medical Form is required for all campers

(except Summer Splash).

There are 3 ways to access and provide this information:

1. Login to the Medical Form process. You will need your MyCampIAWAH login username and password. Fill out Medical Forms now.

To enter information for multiple children you will need to login each time and select the camper you are entering the information for (this is for process and security reasons).

2. Download a paper copy of the Medical Form and then mail or fax it back. Address: Camp IAWAH 304 IAWAH Rd RR2 Godfrey ON  K0H 1T0; Fax: 613.273.3487. Download the Medical Form now.

3. Email or phone and request a Medical Form to be mailed to you. Email: info@iawah.com; Phone: 613.273.5621.
